How We Deliever Care
Our 6 Step Process
#1 Initial Consultation
A care coordinator or representative gathers essential information about the client's needs, preferences, and the type of care required. A preliminary assessment is conducted to understand the scope of the services needed and the client's unique situation.
#2 Care Plan Development
Based on the initial assessment, a personalized care plan is developed, addressing the specific requirements and goals of the client. The plan outlines the type of care services to be provided, the schedule, and any specific considerations related to the client's condition or situation.
#3 Client Consultation and Agreement
The care plan is presented to the client or their family for review and discussion. Any adjustments or modifications to the plan are made based on the client's feedback and preferences. Once both parties are in agreement, a contract or service agreement is signed, outlining the terms and conditions of the care arrangement.
#4 Carer Assignment and Training
We will assign a suitable carer based on the client's needs and compatibility. Carers undergo comprehensive training to ensure they are well-equipped to provide high-quality care and have been DBS checked to ensure security and safety protocols are met. Specialized training is provided for specific conditions or requirements, such as dementia care and medical support.
#5 Service Implementation and Monitoring
The assigned carer begins delivering the care services according to the agreed-upon schedule and plan. Regular monitoring and supervision are conducted to assess the client's progress and ensure the services are meeting their needs effectively. The care coordinator maintains open communication with the client and their family to address any concerns or make adjustments to the plan as necessary.
#6 Ongoing Support and Review
We will provide ongoing support and availability for any additional needs or emergencies that may arise. Periodic reviews are conducted to evaluate the effectiveness of the care and make improvements if needed to ensure client and family satisfaction. Any necessary adjustments to the plan will also be made to meet changing needs over time. We also feedback this information into our internal processes to continually improve our quality assurance systems.
